West Lothian Community Health and Care Partnership (CHCP)
West Lothian was the first area in Scotland to introduce a Community Health and Care Partnership.
The vision of the partnership is to enhance and develop the delivery of integrated health and social care services to the people of West Lothian.
Our aims:

- Reduce life inequalities
- Maximise resources Reduce bureaucracy
- Strengthen service delivery
Jointly managed by and accountable to NHS Lothian and West Lothian Council, the CHCP will co-ordinate the planning, development and provision of better and more efficient health and care services. Senior managers are co-located at the Strathbrock Partnership Centre in Broxburn.
The work of the CHCP is governed by a partnership board and consists of four elected members and four NHS Lothian representatives.
Core services of the partnership are community care services and related hospital services. The partnership covers mental health, children services, learning and physical disabilities, residential, continued and personal care, dental, optical and general practitioner services.
